In the state of California, vital records are categorized as public information. Aside from the procedures and requirements, the website also provides downloadable application forms and pamphlets in PDF format, which you will submit together with a notarized sworn statement if you wish to obtain an authorized copy of any California death records and vital information. The sworn statement, however, is not required for informational copies.
